Berry, Jesse  L.-- "Aqueous Humor is Superior to Blood as a Liquid Biopsy for Retinoblastoma" 
  • Summary:  Purpose: to evaluate whether tumor-derived cell-free DNA (cfDNA) can be isolated as effectively from the blood as aqueous humor (AH) in retinoblastoma (RB) eyes. Methods: AH was extracted via clear corneal limbal paracentesis at diagnosis or during intravitreal injection of chemotherapy. Matched venous blood samples were drawn. Shallow whole genome sequencing was done to assess cfDNA fractions and somatic copy number alterations (SCNAs) in blood and AH samples. Results: Five samples of AH extracted at diagnosis and 13 during intravitreal injection were compared to matched blood samples from 16 patients (2 bilateral patients). The presence of detectable SCNAs in AH was 14/18 and 0/16 in blood. Size distribution of cfDNA in the AH peaked at 157 bp versus 184 bp in the blood. Conclusions: Based on size profiles of cfDNA and absence of SCNAs in the blood, the AH appears a superior source of tumor-derived cfDNA than blood and thus a better target for development as a liquid biopsy.

Chiu, Cynthia S. -- "Steep Learning Curve with Intraoperative Aberrometry"  (NEW)
  • Summary:  Intraoperative aberrometry is a valuable tool available to improve the refractive outcomes in cataract surgery. However, as with many new surgical technologies, a surgeon's early experience may be fraught with challenges and inconsistent results. Of all the tools I use in refractive cataract surgery, I find ORA to have been the most difficult to master, and I would like to share my experiences and surgical results with the group, in hopes of aiding others who are interested in/learning to use this important technology.

Cole, Charles  -- "Perils of Innovation: Tip of the Iceberg?" 
  • Summary:  Surgical devices and techniques are expanding at an unprecedented pace. Unforeseen problems can crop up with any new device or procedure. This has led to one product withdrawal, but procedures can also have unanticipated consequences. A very common and increasingly popular secondary IOL procedure has been described by Shin Yamane, MD. I describe a case of a late haptic disinsertion from the IOL optic body after placement using the "Yamane" technique.

Coney, Joseph M.  -- "Reduction of Treatment Burden in Diabetic Macular Edema" 
  • Summary:  We performed a retrospective study of treatment with the fluocinolone acetonide implant (FAc, Iluvien) for diabetic macular edema (DME) in patients in a real-world setting. The study assessed injection burden, efficacy, and safety before and after treatment with a single FAc implant. The results showed that all patients experienced stable or improved VA, reduced foveal thickness, and a reduction of injection burden.

Cunningham, Matthew A.-- "Full-thickness Macular Hole Formation after Repair of Rhegmatogenous and Tractional Retinal Detachment" 
  • Summary:  The objective of this study is to report the clinical features, associations, and outcomes of patients with a full-thickness macular hole (MH) formation after pars plana vitrectomy for retinal detachment (RD). This retrospective, interventional case series of consecutive patients was performed from September 2014 to October 2018 at a single vitreoretinal surgery practice. A total of 996 cases of rhegmatogenous and tractional retinal detachment repair were identified. The incidence of subsequent MH formation, in patients that underwent surgery, was 0.8% (8/996). In cases with at least 3-months of postoperative follow-up, MH closure was achieved in 100% of eyes after a single surgery. The mean preoperative visual acuity (VA) was 2.06 logMAR units (20/2296 Snellen Equivalent) and significantly improved to 0.72 logMAR units (20/104 Snellen Equivalent; p=0.017), at the last follow-up after MH repair. Overall, all patients had improvement in VA at the final postoperative visit with a VA of 20/200 or better achieved in 87.5% of cases.

  • Summary:  Bilateral sequential same day cataract surgery has been growing in acceptance both nationally and abroad due to a number of factors including healthcare cost and patient preference, however a number of barriers still prevent this from being mainstream in the United States. I will describe some of the advantages and disadvantages of bilateral same day surgery, and my experience with this protocol.

Epley, David -- "ABO MOC FYI 
  • Summary:  I will present a brief update from the ABO regarding the status of Maintenance of Certification for ophthalmologists who are board certified and participating in the MOC process. The talk will address the ABO's efforts to make the process less onerous and better integrated into the daily flow of a busy ophthalmology practice.

Frankfort, Benjamin-- "Mouse Retinal Ganglion Cells are Severely Impacted by Small IOP Increases: New Important Clinical Considerations" 
  • Summary:   We know incredibly little about the actual, direct, immediate impact of IOP increases on retinal ganglion cells. Recent mouse research from my laboratory has shown that even small IOP increases can have profound effects on retinal ganglion cell function and gene expression. I will present and discuss these findings, as well as their potential clinical impact.

  • Summary:   The World Ophthalmology Congress is a great Ophthalmology meeting attended by Ophthalmologists all over the world. In 2020, it will be in South Africa. This is the first time the meeting has been held on the African continent. In my talk, I will discuss several reasons why AESC members should attend.

  • Summary:   SMILE Vision Correction is a relatively new laser vision correction procedure. I will show a new technique that I use to extract the SMILE lenticule without using a forceps. I call it an "Immaculate extraction."

  • Summary:    Patients have been harmed and physicians have been sued when the physician did not realize a relationship had been established or termination of the relationship was not handled appropriately. A plaintiff cannot prove negligence if there is no doctor-patient relationship. Education about the duty of care and management of the physician-patient relationship will be provided through a series of case presentations and discussion.

Hasan, Sohail -- "Anterior Segment Surgery-The Retina Perspective" 
  • Summary:    This presentation focuses on the retinal surgeons approach to subluxated and dislocated PCIOLs. Risk factors, troubleshooting and surgical planning are discussed with respect to sunrise/sunset syndrome and posterior dislocation of PCIOLs. Case examples are given with associated surgical videos. The presentation ends with a complex post surgical scenario that undergoes corrective surgery.

Horn, Erich P. -- "How did the implementation of Medicare affect Ophthalmologists?" 
  • Summary:   Medicare, the federal social insurance program which pays for hospital and medical care services for Americans age 65 and older, was passed by Congress and signed into law by President Lyndon Johnson in 1965. While politically popular, the legislation was opposed by many physicians and their professional organizations. The effect of this landmark legislation on the private practice of ophthalmology has been little studied. By examining the practice financial records, patient logs and recollections of office staff and surviving ophthalmologists and their spouses through the archives of the Alachua County Medical Society, I will illustrate the effect of Medicare on particular ophthalmologists in north-central Florida which may help generalize the effect on the larger group of American ophthalmologists. I hypothesize that patient volume increased over the ensuing three years after implementation, that ophthalmologists' income rose and that difficulties with collection of fees from elderly patients were diminished.

Kipp, Michael -- "Trends in Adult Strabismus: One Man's Perspective" 
  • Summary:   A review of my first twenty years in practice has shown an increase in adult strabismus surgery patients. This seems to be a nationwide trend. Acquired esotropia appears to account for the majority of this increase. Possible mechanisms for this increase will be reviewed. My personal use of adjustable sutures has decreased over this time period, and reasons for this will be discussed.

Kishor, Krishna -- "Is an OCT better than an exam?
  • Summary:   At times the exam is inconclusive with regards to a diagnosis of glaucoma. Utilizing a case, I will illustrate that using the history, exam and diagnostic tests can help diagnose/confirm glaucoma in borderline cases.

  • Summary:   Surgery for trachomatous trichiasis remains a primary strategy behind the World Health Organization's goal to eradicate trachoma globally by 2020. This update will review the global burden of disease, the role and techniques of surgical management, and emerging innovations that have the potential to "flip the script" in the battle to restore sight on a large scale in much of the world.

  • Summary:   A case of bilateral uveitis in a patient on systemic immunosuppression with HZO will be presented with a discussion of the differential diagnosis and management course. A review of recent literature on risks of infection in patients on systemic biologic therapy and data on ocular complications in HZO will be presented.

  • Summary:  A systematic assessment of the adult with diplopia complaints will be reviewed along with surgical and non-surgical management. There have been recent advancements in the management of orbital inflammatory conditions with the introduction of new immune suppressants. Their role in managing the adult patient with diplopia complaints will be discussed along with surgical considerations made in a few case-by-case examples. This discussion will focus on the adult patient with thyroid eye disease-related diplopia.

Raju, Leela -- "HSV Keratitis Recurrence Rates with Topical Prophylaxis 
  • Summary:  Twenty thousand new cases and 28,000 recurrences of herpes simplex occur in the US each year. HEDS demonstrated that oral acyclovir reduces recurrence. However, no studies indicate whether adding a topical corticosteroid can further reduce this risk ¿ specifically in stromal keratitis or keratouveitis. A retrospective review of patients suggests that long-term, low-dose topical corticosteroids have a place in the prevention of recurrent HSV stromal keratitis or keratouveitis.

Robbins, Shira -- "The MOST Exciting Thing in Pediatrics: Ocular Gene Therapy" 
  • Summary: We are in the Jetson era. Treatments that we only imagined are becoming reality. When I worked my 1st job after college in a molecular oncology lab sequencing DNA, the concrete reality of using genetic therapies seemed very far away. This talk will cover the usage of gene therapy for pediatric eye diseases.

Rosenblatt, Mark-- "Seven Things I wish I Knew Before Becoming a Chair" 
  • Summary: Physician leadership has always been an important part of organization success. Increasing financial and regulatory stresses on medicine have made the roles of physician leaders more difficult, just as the need for leadership has become even more critical. Almost five years ago, I assumed the role of department chair, an important and challenging role in academic ophthalmology. In many ways this mirrors the role of physician leaders in the private practice environment. In my talk I will share some of the lessons I learned as a new physician leader, to provide information to those who mas aspire to leadership roles in ophthalmology, but also to give context for those who interact with physician leaders in their own organizations.

Schwartz, Stephen G.-- "Famous Monocular Warriors 
  • Summary:  The loss of an eye can be associated with many negative and lifelong consequences to a patient. Interestingly, some of the most successful military commanders in history have been monocular. These include Philip II of Macedon, Federico III da Montefeltro (my personal favorite, and the subject of a previous AESC talk), and perhaps Horatio Nelson, although the evidence in support of Nelson's eye injury is questionable. Their life stories are interesting and may prove inspiring to ophthalmologists and to our patients.

Villegas, Victor M.-- "Peeling Conundrum 
  • Summary:  15 month-old child without systemic disease was consulted to the ophthalmology service 3 months after a severe motor vehicle accident. Immediately following the accident, the patient developed increased intracranial pressure secondary to an intracranial hemorrhage and underwent a craniotomy. Upon ophthalmologic evaluation the patient was found with bilateral vitreous hemorrhage and intraretinal hemorrhage. Terson's syndrome was diagnosed.   • Summary:  Recently my practice experienced a perceived increase in postoperative pseudophakic CME using a one-piece acrylic lens. Results of our three ophthalmologist practice since transition to a new ASC using different implants and intracamaral antibiotics will be presented.

  • Summary:  We will review the treatment options for neutrophic keratitis including the use of Oxervate. Oxervate is a new FDA-approved eye drop use to treat this chronic condition. We will review our clincal experience with this drug.
